Medication Policies
If you need to bring in medication for your student, we encourage you to complete the form(s) below prior to your arrival. We hope that this will expedite the amount of time it will take you to drop off medication.
The health office would like to remind you that there are specific state laws in Oregon regarding students taking medication at school (prescription or non-prescription). This is for your student's safety.
Here are some important rules to understand regarding self administration and school administration:
- 1. To administer prescription medication, we must have a valid prescription label from the pharmacy.
- All medications (prescription and non-prescription) must come in the original packaging.
- Under no circumstances can a student share medication with another student.
- If a student is going to self-carry medication they are only allowed to carry enough for a single day (inhalers are the exception).
Self Administration
- A student is allowed to self-administer non-prescription medications when they are able to demonstrate the ability, developmentally and behaviorally, to self medicate and has permission to self-medicate from a school building administrator.
- A student is allowed to self-administer prescription medications when they are able to demonstrate the ability, developmentally and behaviorally, to self administer medication and have permission to self-administer from a school building administrator, and a prescriber or registered nurse practicing in the school setting.
School Administration
- To administer prescription medication, school personnel must have a written request from the student's parent/guardian which includes signed permission for school staff to give the medication.
- Written instruction includes: (1) Name of student (2) Name of medication (3) Method (by mouth, etc.) (4) Dosage (5) Frequency—how often it is to be given
