Allergy Protocol

Our kitchens process all major allergens. Due to supply chain constraints, we are also receiving substitutions from our vendors without notice. If you have a food allergy, please connect with your school nurse.

1. All students with food allergies should fill out the Severe/anaphylactic allergy questionnaire.
   
   
2. Any student with food allergies who require a SEPARATE MEAL different than was offered, needs a physician to fill out this form . Alternative languages are located here.
   
   
3. Our goal in nutrition services is to connect with every student who needs a separate meal, letting families know the risks involved. 
   
   
4. Secretaries, please share with counselors, other secretaries, and those who may receive allergy questions.
